Dr. Kimball is in-network providers for the following insurance plans:
Dr. Kimball is out-of-network providers for these plans:
Many insurance companies will reimburse some portion of the cost to out-of-network providers. You pay at the time of service and we immediately provide you with a superbill to give your insurer.
Your insurance is an agreement between you and your insurance company, not between your insurance company and our office. We cannot be certain if your insurance covers naturopathic care, and the amount they pay varies from one policy to another. Please call your insurance company and make sure you understand what your coverage is.
We will bill your insurance company for the service you receive. You are responsible for copays. If your deductible has not been met you are responsible for the cost of the visit. Insurance does not always cover all office visit charges nor laboratory charges.
Services not covered by insurance are charged to you directly and you are personally responsible for payment of any such services, deductibles, co-pays or co-insurance.
Flex Plans/ Medical Savings Account/ Health Savings Accounts
Please inform us if you have a Health Savings Account, sometimes known as a Flex Plan or Medical Savings Account. All naturopathic medical care, including co-pays, deductibles, co-insurance, lab, and prescribed supplements can be paid with funds from your Flex Plan/MSA/HSA.
Please inform us of any secondary insurance you may have, though our office is not responsible for submitting claims to your secondary insurance provider.
Patients without Insurance
For patients without insurance, payment for your visit (75-90 minutes) is $195.00 and due at time of service. For follow-up visits, we offer a "time of service" discount based on time and the complexity of your visit. We accept cash, your check, or credit card. Please call at 206.937.6747 for more specific pricing information.
Rescheduling & Cancellation
We charge $50 for missed visits or those not cancelled two business days before appointment time. This fee is not billable to insurance. We need to have this policy because missed appointments ultimately drive up the cost of healthcare for everybody. We are setting the time aside specifically for you.
Supplements are an extra cost and are added to the appointment costs. These can be paid through HSA/MSA/Flex Plans and are tax-free when prescribed by our physicians.
Many lab tests are covered by insurance, some are not. These can be paid through HSA/MSA/Flex Plans when not covered.
